This is case of 14 year old female came with chief complaints 
1.Fever since 4 days
2.Headache since 4 days

HOPI -

Patient was apparently asympomatic 4days back

Then she developed 

Fever which is of high grade and onset relived with medications,associated with chills and rigors

Headache since 4days  diffuse in region.

No history of cold,cough 

Vomiting since yesterday morning 3-4 episodes 

Abdominal pain since yesterday morning

No k/c/o DM/HTN/Epilepsy/CAD/Asthma

TREATMENT HISTORY -
Not significant

Diet-Mixed
Appetite-Normal
Sleep-Adequate 
Bowel and bladder-Normal
Micturtion-Normal

No Addictions

1.IVF NS and  RL  @75ml/hr

2. INJ PAN 40mg IV/OD

3.INJ ZOFER 4mg/IV/TID

4.INJ NEOMOL 1gm/IV/SOS

5.TAB PCM 650mg PO/QID

6.INJ TRAMASIL 100mg/IV/SOS
